Youth Villages provides dental insurance through Delta Dental. Dental insurance is available for employees and their families.
- Annual one-time deductible: $50
- Annual coverage: $1,000
- Lifetime orthodontic maximum: $1,000
Orthodontic coverage applies to children of employees only, not employees. Children must be under age 19.
Dental coverage includes:
- Oral exam, cleaning and fluorides (two in 12 months; fluoride up to age 19)
- Prophylaxis
- Bitewing X-rays
- Full-mouth X-ray (one per 36 months)
- Root planning and scaling (one per quadrant in 24 months, does not cover occlusal guard)
- Space maintainers (for missing posterior, primary teeth to age 14)

The qualified company-sponsored retirement is a defined contribution plan administered by a contract administrator. Youth Villages makes annual contributions to the plan. Contributions are allocated to an individual account for each employee who is a participant in the plan.
An employee earns a vesting year of service when he/she works 1,000 hours of service in a plan year. The vested percentage is based on years of service as shown on the schedule below:
- 1 year of service - 0% vested
- 2 years of service - 20% vested
- 3 years of service - 40% vested
- 4 years of service - 60% vested
- 5 years of service - 80% vested
- 6 years of service - 100% vested
The contribution shall be determined by the board of directors on an annual basis, and a percentage shall be set as the basis of compensation.
Compensation shall be defined as total wages on salary paid including overtime or bonuses but excluding any expense allowance not taxable to participant.
No hardships loans under any circumstance.

Youth Villages offers tax-deferred savings plans for retirement through a 403b plan. There are three different plan options you may sign up for at any time. They are offered through:

Youth Villages is committed to providing great health care at affordable prices to our staff. Full-time Youth Villages employees are insured through United Healthcare, the nation's largest health care provider, at a minimal monthly cost that is salary-deducted. Health care becomes effective after three months of full-time employment.
Co-Pay
Office visits for preventive care and visits to specialists:

Network: $20 per physicial/specialist visit (includes physical, eye exam and refractive eye exam every 24 months by in-network provider. Routine mammography payable in full)

Office visits for illness:
- Network: $20 per physician/specialist visit. Diagnostic lab work, X-rays are covered at 80% (subject to deductibles and out-of-pocket maximum)
- Out-of-network: Covered at 60% (subject to reasonably customary limits)

Medication coverage is provided by Medco. Medication is divided into three tiers of cost. The insured may either order a 30-day supply of medication at the pharmacy or order a 90-day home-delivery supply through Medco to receive medication at reduced cost.
Pharmacy pick up: 30-day supply

- Tier 3 - $40
Mail/home delivery: 90-day supply
- Tier 1 - $20

Oral contraceptives and male erectile dysfunction (ED) medication are covered, but ED coverage is limited.

In-patient Services
- Network: 80% coverage (subject to deductible and out-of-pocket maximum)
- Out-of-network: 60% coverage (subject to deductible, out-of-pocket maximum and reasonable and customary limits for hospital, maternity and surgical procedures)
Emergency Room
- Network: 80% coverage for emergencies only (subject to deductible and out-of-pocket maximum)
- Out-of-network: 80% coverage for emergencies only (subject to deductible and out-of-pocket maximum) - patients must notify claims administrator within two business days to receive network coverage
Home Health Care & Skilled Nursing Care
- Network: Covered at 100% after deductible. Services are limited to 100 days per calendar year. Patients must notify Care Coordination within five business days to ensure full coverage
- Out-of-network: Covered at 80% after deductible. Services are limited to 100 days per calendar year. Patients must notify Care Coordination within five business days to ensure full coverage
Mental Health Services
Any combination of network and out-of-network mental health services is limited to 30 visits per calendar year.
In-Patient:
- Network: covered at 80% after deductible and out-of-pocket maximum
- Out-of-network: covered at 60% after deductible and out-of-pocket maximum (subject to reasonable and customary charges)
Out-patient:
- Network: covered at 100% for 3 sessions through LifeBalance; covered at 80% after deductible has been met
- Out-of-network: covered at 60% after deductible and out-of-pocket maximums (subject to reasonable and customary charges)
Substance-Abuse Treatment
Any combination of network and out-of-network services for substance-abuse treatment is limited to 30 visits per calendar year.
In-Patient:
- Network: covered at 80% after deductible (up to out-of-pocket maximum); authorization required
- Out-of-network: 60% coverage after deductible; authorization required
Out-Patient:
- Network: covered at 100% through Employee Assistance Program (3 sessions per year), 80% after deductible; authorization required
- Out-of-network: covered at 60% after deductible; authorization required

Full-time (40 hrs.) and regular part-time (30 hrs.) Youth Villages employees automatically have life insurance through Unum. Accelerated benefits, conversion and portability apply.
Life insurance:
- Flat employer-paid coverage: $15,000
Additional coverage:
The amount of life insurance payable depends on the age of the insured, as well as the age at which coverage becomes effective.
- For ages 70-74, the additional coverage is 65% of insured's coverage from prior to age 70; or
- For people who become insured at age 70-74, 65% of amount shown above
There will be no further increases in life insurance.
- For ages 75 and above, additional coverage is 50% of insured's coverage from prior to age 75; or
- For people who become insured at age 75, 50% of amount shown above
There will be no further increases in life insurance.
Accidental Death & Dismemberment Benefits (AD&D)
AD&D benefits are the same as the life insurance benefits described above. AD&D flat rate is $15,000 paid by the employer. As with life insurance, the amount of AD&D insurance payable depends on the age of the insured, as well as the age at which coverage becomes effective (see life insurance above).
Education Benefit
Education benefits are provided by Unum should the insured pass away due to an accident. In that case, the insured's child is entitled to education benefits only if the child is either a senior in high school or is attending college.
Education benefits are separate from any AD&D benefits. AD&D benefits must be paid first for the qualified child to become entitled to education benefits.
Education benefits includes:
- the lesser of 12.5% of the full amount of the employee's AD&D benefit; or
- $12,500
- total education benefit could equal: $100,000
Education benefits may be paid in eight installments.
Seatbelt/Air Bag Benefits
Seatbelt and air bag benefits are separate from any AD&D benefit that may be payable. AD&D benefits must be paid first for seatbelt/air bag benefits to apply. Benefits include:
- $10,000 if the insured dies as the result of an accident in a private passenger car but the person was wearing a seatbelt
- $15,000 if the insured was protected both by a seatbelt and air bags
Repatriation Benefit
UnumProvident will pay up to $5,000 to help defray the cost of moving the body of the insured if the accidental death occurs at least 75 miles from the insured's primary residence.
The Repatriation Benefit is separate from any AD&D benefits. AD&D benefits must be paid first before Repatriation Benefits to become available.
Voluntary Term Life Insurance
Employees are eligible to purchase up to $150,000 of coverage on a guarantee-issue basis (no medical unterwriting) if this is the employee's first time participating in the program.
An employee may also purchase guarantee-issue coverage of up to $25,000 for his or her spouse and up to $10,000 for dependent children.

Short-Term Disability Insurance
Short-term disability insurance replaces a portion of an employee's monthly income in case of illness or when an employee's off-the-job injury prevents the employee from fulfilling his or her job duties.
Employees may choose the amount of coverage ($400 per month to 50% of monthly salary) and beginning date of coverage (after seven or 14 days).

Accident Insurance
Fact is, accidents do happen, and most of them happen close to home, not at work. Employees between the ages of 21 and 80 who work a minimum of 20 hours per week and who have been employed with Youth Villages for at least one year qualify for accident insurance. This supplemental insurance provides extra coverage for accidents that happen outside of work or at home.
Critical Illness Insurance
A severe and even life-threatening illness is not only frightening but can drain finances fast. Critical Illness Insurance pays a lump sum to a severely ill employee who has been diagnosed with one of the following: heart attack, stroke, major organ transplant, permanent paralysis, kidney failure, bypass surgery or cancer (must be elected separately).

Youth Villages provides 100% tuition reimbursement and 100% licensure reimbursement up to $8,000. The purpose of this program is to financially assist employees in pursuing accredited higher education and/or licensure that is directly related (as determined by the employee's supervisor and/or director) to their professional job function at Youth Villages.
Regular full-time employees (40 hrs.) may participate in the tuition reimbursement program after 6 months of continuous employment. Courses must be approved by Youth Villages, with a limit of two courses per semester or per quarter. Tuition will be reimbursed at 100% at certain state universities; at others with higher tuition rates, Youth Villages will reimburse a portion of tuition. Tuition will be reimbursed if the employee maintains a B average for master's-level classes and a C average or higher for bachelor's-level classes.

On-site Fitness Centers
The Operations Center features a brand-new fitness room with two treadmills and an elliptical trainer, as well as free weights.
The Bartlett Campus also has a new exercise room with treadmills, a stationary bike and weights. The campus also has a lake with a walking trail.
The Dogwood Village Campus has on outdoors walking and running trail.
Gym Discounts
Youth Villages employees in the Memphis area may join the Well Works Sportsplex at 20% off the regular membership price. Membership dues must be paycheck-deducted. Members must send in receipts monthly or quarterly.
Youth Villages employees outside of Memphis may join a YMCA in their area at a 20% discounted rate.
Weight Watchers
Weight Watchers groups are organized by staff and held at a local office. Participants pay the registration fee plus the fee for the initial eight-week membership. Employees will be reimbursed for their initial 10-week membership fees if they lose 10 pounds or more at the end of the 10 weeks. Youth Villages will reimburse the registration fee and continue to reimburse employee membership fees as long as the employee continues to lose weight.

The LifeBalance program is provided through the Assist America Program. LifeBalance is a confidential assistance program to help with all the issues of daily living. Assist America is a worldwide emergency assistance service for travelers. This benefit is included at no additional cost.
